Greig Health Record (Age 6-17)

The Greig Health Record is an evidence-based health supervision guide for healthcare providers for children and adolescents aged 6 to 17 years.  It is endorsed by the Canadian Paediatric Society (CPS), and the College of Family Physicians of Canada (CFPC). This document provides checklists in the following domains:

  • Growth and nutrition monitoring
  • Psychosocial history and development (HEADSSS)
  • Physical examination parameters
  • Immunizations
  • Anticipatory guidance on safety, family, behaviour and health promotion issues

By using the Greig Health Record you can structure a well-child visit for school age children and adolescents and ensure that you review key items that are validated by research to ensure optimal child health.
